E. IDENTIFICATION & ACCOMMODATION

BPT’S PROCEDURES FOR COMPLIANCE

  • The BPT has developed BPT 1073 form (Identification and Notice of Reasonable Request for Accommodation) to identify prisoners & parolees with disabilities, to notice prisoners/parolees of the rights under the ADA, and to serve as a tool for requesting reasonable accommodation.

  • Prisoners/parolees are provided with a BPT 1073 form prior to a parole proceedings.

  • Staff or counsel providing a prisoner/parolee with a BPT 1073 shall assist the prisoner/parolee in completing the form when they are unable due to a disability.

  • The exact content of the statement on Section II of the BPT Form 1073 must be effectively communicated to a prisoner/parolee.
