BPT 1074 FORM: 

There are two levels of Grievance review and disposition: 

First Level Review: BPT ADA Coordinator
Second Level Review: Chief Deputy Commissioner

The BCPA will review the BPT 1073 with the parolee and determine if the accommodation requested is reasonable.

During the due process review, the BCPA will review the BPT Form 1073 with the inmate/parolee and explain the grievance process via the BPT 1074 Form. The BPT Form 1073 review will be documented on a BPT Form 1073(b), Due Process/ADA Review.

First Level Review - If the inmate/parolee is not satisfied with the accommodation provided, or if the BCPA denies the request for reasonable accommodation, the BCPA will provide the inmate/parolee with a BPT Form 1074. The 1074 shall be faxed to the ADA Coordinator and a decision shall be rendered. 

Final Level Review - if the I/P is unsatisfied with the ADA Coordinator’s decision, the Chief Deputy Commissioner will make the final decision.
