BPT 1073 FORM: Notice and Request for Reasonable Accommodation

The Board will utilize BPT Form 1073 to notify inmates/parolees of their rights under Title II of the ADA prior to a parole proceeding.

Allows inmates/parolees to request an accommodation.

The BPT 1073 (Notice and Request for Reasonable Accommodations) serves several purposes in compliance with the Injunction:

  • Notice of ADA Rights

  • Request for reasonable accommodation

  • Opportunity to self-report

  • Identification of Disability

  • Tracking of inmates/parolees with disabilities

  • Documentation of effective communication

  • Interactive process

It is the responsibility of the prisoner/parolee to request a reasonable accommodation in order to ensure effective communication and/or request access by correctly completing the BPT 1073. 
BPT /CDC staff will also identify and make recommendations for persons they assist/observe that need a reasonable accommodation.
