BPT 1073/1074 FORM: 

An inmate’s/parolee’s request will be given primary consideration. The BPT makes the final determination and approval for reasonable accommodation requests. 

The inmate/parolee will be advised that he/she will have the right to grieve the decision (if adverse) of the request via a BPT 1074 Form for reasonable accommodation and advise parolee that a decision will be made prior to a scheduled hearing or parole proceeding 

Prisoners/parolees may wish to file a grievance based on the denial of their request for reasonable accommodation . All ADA-related complaints of denials for an accommodation are handled via the BPT 1074 Form. The inmate/parolee must submit the BPT 1074 Form within five days from the date the request was denied (date of BPT Due Process Review).
